Mixoma de ventrículo derecho simulando un trombo en una paciente con lupus eritematoso y síndrome antifosfolípido. Claves para un correcto diagnóstico y tratamiento

Se reporta el caso de una paciente con Lupus Eritematoso Sistémico (LES) asociado a síndrome antifosfolípido (APS), a quien por medio de un ecocardiograma transtorácico de despistaje de trombos dentro del aparato cardiocirculatorio se encontró una imagen ecodensa en cavidad ventricular derecha. Con la ayuda del ecograma transtorácico pudo ser mejor delimitada planteándose el diagnóstico de un mixoma y no de un trombo como inicialmente se consideró. Se practicó una ventriculotomía con resección del tumor, el cual, de acuerdo al análisis histopatológico, fue confirmado como un mixoma. Este caso pone de relieve la importancia crucial de realizar evaluaciones ecocardiográficas rutinarias en pacientes con LES y APS concomitantes. En este contexto, el ecograma transesofágico permite una mejor delimitación de la masa y establecer la indicación para una ventricu- lotomía.
We report the case of a 20-year-old female patient with systemic lupus erythematosus (SLE) associated with antiphospholipid syndrome (APS) in whom, by means of a transthoracic echocardiogram for thrombus clearance within the cardiocirculatory system, an echodense mass was found in the right ventricular cavity. By means of a transthoracic echogram the mass could be better delimited, leading to the diagnosis suspicion of a myxoma and not a thrombus as initially thought. A ventriculotomy was performed with resection of the tumor, which, according to histopathological analysis, corresponded to a myxoma. This case highlights the crucial importance of performing routine echocardiographic evaluations in patients with concomitant SLE and APS. In this context, transesophageal echogram allows a better delineation of the mass and may establish the indication for ventriculotomy.
