Correlación entre aclaramiento de creatinina y fórmulas predictivas en consulta externa de CMFIEDM, abril-julio 2019

El filtrado Glomerular es un marcador fundamentalmente aceptado para la evaluación de la función renal. Como este marcador no se puede medir de forma directa, por lo que se han desarrollado diferentes mecanismos de medición como el Gold Estándar que es el aclaramiento de creatinina y las fórmulas predictivas. Las fórmulas predictivas estiman el filtrado glomerular con ayuda de la creatinina sérica peso, edad, género y etnia del paciente. La intención de la presente investigación fue valorar el nivel correlación que tienen las fórmulas predictivas frente al aclaramiento de creatinina; para lo cual se realizó un estudio prospectivo, transversal, relacional el cual consistió en obtener la orina de 24 horas de los pacientes que asistieron a consulta externa en el periodo abril-julio 2019; y además se tomó las medidas antropométricas como el peso, edad, género y etnia y se procedió a calcular el aclaramiento de creatinina y aplicar las fórmulas predictivas. Por medio de estos parámetros se determinó el coeficiente correlación de Spearman el cual obtuvo para la fórmula Cockcroft-Gault 0,556; MDRD-4 fue de 0,518 y CKD-Epi fue 0,555; los coeficientes establecieron que poseen una correlación moderada frente a los valores del aclaramiento de creatinina; por lo que se considera que las utilización de las fórmulas predictivas es válida para la que se pueda determinar la tasa de filtración glomerular para la validez clínica y diagnóstica dentro del laboratorio clínico o en una de emergencia.
Glomerular filtration is a fundamentally accepted marker for the evaluation of renal function. As this marker can not be measured directly, so different measurement mechanisms have been developed such as the Gold Standard which is creatinine clearance and predictive formulas. Predictive formulas estimate glomerular filtration with the help of serum creatinine weight, age, gender and ethnicity of the patient. The intention of the present investigation was to assess the level of correlation that the predictive formulas have against creatinine clearance; for which a prospective, cross-sectional, relational study was conducted which consisted of obtaining 24-hour urine from patients who attended an outpatient clinic in the period April- July 2019; and in addition, anthropometric measures such as weight, age, gender and ethnicity will be taken and the creatinine clearance will be calculated and the predictive formulas applied. By means of these parameters the Spearman correlation coefficient was determined which obtained the Cockcroft-Gault formula 0.556; MDRD-4 was 0.518 and CKD-Epi was 0.555; the stably coefficients that have a moderate correlation against creatinine clearance values; Therefore, it is considered that the use of predictive formulas is valid for which the glomerular filtration rate can be determined for clinical and diagnostic validity within the clinical laboratory or in an emergency.
