Organophosphates are the most common cause of pesticide poisoning worldwide. It has been estimated that there are about 3,000,000 cases of organophosphate poisoning each year, with 300,000 fatalities (Bird, 2014). In our country, the annual reports issued by CIATOX indicate the existence of 1,243 cases of pesticide poisoning in 2011, 1,605 in 2012 and 1,970 in 2013; being the leading cause of poisoning its intentional ingestion (Solis, 2013). With the information above we can infer how important it is for the health personnel in training to learn about organophosphate poisoning and to be capable of providing adequate care to these patients. In this paper, we analyze the case of a young patient who suffered severe acute poisoning due to the intake of an organophosphate and a vitamin K antagonist, which required complex management in a hospital setting. Based on this case, we will go through the available medical literature while emphasizing the fundamentals of diagnosis, initial management and specific treatment. The information will be summarized in an interactive presentation to capture the reader's interest and facilitate his/her learning.
Los organofosforados constituyen la causa más frecuente de intoxicación por plaguicidas a nivel mundial. Se ha estimado que ocurren alrededor de 3.000.000 intoxicaciones por organofosforados al año, con 300.000 casos fatales (Bird, 2014). En nuestro país, los reportes anuales emitidos por CIATOX indican la existencia de 1.243 casos de intoxicaciones por plaguicidas en el 2011, 1.605 en el 2012 y 1.970 en el 2013; siendo la principal causa la ingesta intencional (Solis, 2013). De ahí la importancia de que el personal de salud en formación conozca de esta intoxicación y esté en la capacidad de ofrecer un manejo oportuno a estos pacientes. En el presente trabajo se analiza el caso clínico de una paciente joven con un cuadro de intoxicación aguda grave por ingesta de organofosforado y antagonista de la vitamina K, que requirió manejo hospitalario complejo. En base al caso, se hará referencia a la literatura médica disponible, enfatizando en los aspectos fundamentales del diagnóstico, manejo inicial y tratamiento específico. Se resumirá la información en una presentación interactiva con el fin de capturar el interés del lector y facilitar su aprendizaje.
